Registration process:

1.       Please complete the “Records Log” document that was sent to you in your confirmation e-mail and fax to 419-791-7029.

2.       Provided you have paid for your training in full and that you have completed a Records Log Document, you will receive an email titled “Authorization to Test.”  This email will include sample questions, your CRFA™ ID number and instructions on how to schedule your exam appointment. Once you have received the email, call the LaserGrade registration number at 800-211-2754 to register for an examination.

3.       The exam is 100 multiple-choice questions. A block of 4.5 hours will be allotted to you at a LaserGrade testing center of your choice. This time block contains: 

a.      15 minutes for a tutorial at the beginning

b.      4 hours for testing
